To claim you need to be eligible for Medicare.

Eligibility is limited to weighted silicone external breast protheses. This includes silicone breast forms, swim forms, partial forms and shapers.

There is no time limit from surgery to first claim, but subsequent claims can only be made 2 years after the date of last purchase.

Claims can be made in less than 2 years with a letter from a medical professional explaining the medical reason. Contact the STUDIO for appropriate wording (click on phone/email symbol in the top of page grey tool bar).

Up to a maximum of $400 per prosthesis can be claimed, but Medicare will not assess any claim prior to purchase.

Claims cannot be made online. They are lodged with Medicare and can take 3-6 weeks to process.

When the claim is accepted, Medicare pays the reimbursement by Electronic Funds Transfer (EFT) into the bank account nominated on the Claim Form and sends a statement of payment to the nominated postal address.

WHEN YOU DROP-IN TO THE BREASTCARE STUDIO

For a first fitting, we will spend time in the fitting room to create a natural looking breast shape.

For subsequent fittings, we will first review your fitting. It may be that adjusting your bra is all that is needed. If a change in body, breast or lifestyle means that a new shape, size or style is needed, we will fit the new prosthesis. If the prosthesis has been damaged or lost integrity, we will explain any new options and then fit your new prosthesis.

After purchase, we will create a ‘paid’ invoice for the prosthesis and give you a Claim Form to fill out (Medicare Number & Bank Account Details are needed).

We will then email electronic copies of the Claim Form, invoice and medical letter (if applicable) to Medicare on your behalf and ‘cc’ the email to you (if you have given us your email address).

WHEN YOU PURCHASE ONLINE OR BY PHONE

After purchase, we will include a copy of the Claim Form and ‘paid’ invoice with your prosthesis when it is delivered (do not use the web generated invoice).

You then need to lodge the Claim Form, invoice and medical letter (if applicable) using one of the options listed on the form.
